Phacoemulsification and Trabeculectomy: A Comparison of Phacotrabeculectomy and Two Stage Surgery

3Ankara Numune Eğitim ve Araştırma Hastanesi 3. Göz Kliniği Şefi, Ankara Purpose: The comparison of phacotrabeculectomy and phacoemulsification after trabeculectomy in respect to the efficacy and safety.
Materials and Methods: We included the 36 eyes of 33 patients undergone phacotrabeculectomy (group 1) and 23 eyes of 22 patients undergone phacoemulsification after trabeculectomy (Group 2). Of the group 1, 20 had primary open angle glaucoma (POAG) and 13 had pseudoexfoliative glaucoma (PEG) and of the group 2, 14 had POAG and 8 had PEG. The results were evaluated retrospectively and compared statistically. Phacoemulsification and superior trabeculectomy technique with temporal approach was preferred in combined surgery and temporal corneal incision was preferred in phacoemulsification after trabeculectomy. Paired t test, Wilcoxon signed rank test and chi-square test were used in statistical analyses. Statistical significance was accepted as p<0.05.
Results: The mean age was 60.3±6.5 years and 64.5±5.9 years in group 1 and 2 respectively. The follow-up period was 20.8±4.5 months in group 1 and 21.9±9.9 months in group 2. The mean preoperative intraocular pressures (IOP) were 28.4±3.6 mmHg and 27.4±3.5 mmHg in group 1 and 2 respectively. At the last control of the patients the mean IOPs were detected as 15.3±2.7 mmHg and 16.2±2.2 mmHg in group 1 and 2 respectively. The mean number of antiglaucomatous medications was 2.1±0.5 in the first group and 2.1±0.6 before trabeculectomy and 0.48±0.6 before phacoemulsification in group 2. The mean number of antiglaucomatous medications after the surgeries was detected as 0.14±0.5 and 0.52±0.6 in the last control of the patients.
Conclusion: There was no statistically significant difference between the groups in respect to age, sex, the distribution of glaucoma types, the preoperative mean IOPs and follow-up periods (p>0.05). The IOPs were similar (p>0.05), but the number of antiglaucomatous medications were significantly higher in group 2 in the last control of the patients (p<0.05).
