Pseudoeksfoliation and Cornea

Pseudoeksfoliation syndrome (PES) is a common age-related systemic extracellular matrix disorder characterized by the production and accumulation of abnormal fibrillar material in ocular and nonocular tissues. It is the most common recognizable cause of open-angle glaucoma worldwide. Clinical signs of cornea with PES include decreased corneal sensitivity, thinning of the central corneal thickness, and impaired tear film stability and dry eye, decreased corneal hysteresis, pseudoexfoliative keratopathy. deposition of pseudoexfoliation material and pigment on the corneal endothelium.In vivo confocal microscopy studies have demonstrated significant morphologic alterations in patients with PES. The eyes with PES is associated with reduced numbers of corneal epithelium, keratocytes, and endothelium as well as reduction in subbasal nerve density. The viscosity and elasticity of the cornea were lower in PES.
These corneal alterations may have important assessing the impact of surgical interventions of cataract and treatment of glaucoma patients with PES. Further studies are needed to devise a new PES grading scheme for determining the severity of corneal involvement.
