Anterior Chamber and Intraocular Pressure Changes Following Phacoemulsification in Uveitic Patients

Başkanı, Eskişehir, Prof. Dr. Purpose: To evaluate anterior chamber depth (ACD), anterior chamber angle (ACA), and intraocular pressure (IOP) changes following phacoemulsification with posterior chamber intraocular lens (IOL) implantation in uveitic cases.
Materials and Methods: Forty-seven eyes of 39 patients with cataracts due to uveitis underwent phacoemulsification with foldable hydrophobic acrylic intraocular lens (IOL) implantation. IOP values of all cases were below 21 mmHg prior to surgery. In all patients, anterior chamber configurations were normal. Postoperative findings were evaluated every day during the first week, every week during the first month, and then every month until the sixth month after surgery. ACD and ACA values were determined using Orbscan II.
Results: The mean age of the patients was 47.9±19.5 years. The mean preoperative ACD was 2.95±0.36 mm, while postoperatively mean ACD was 3.42±0.41 mm after 1 day, 3.46±0.37 mm after 1 week, 3.51±0.44 mm after 1 month, 3.46±0.35 mm after 3 months, and 3.49±0.33 mm after 6 months (all p<0.01). The mean preoperative ACA degree was 35.56± 3.89°, while postoperatively mean ACA was 43.91±4.28° after 1 day, 44.33±4.17° after 1 week, 44.32v4.11° after 1 month, 44.13±3.84° after 3 months, and 43.89±4.49° after 6 months (p<0.001). The mean preoperative IOP of 18.6±3.5 mmHg did not change significantly postoperatively (19.3±2.4 mmHg after 1 day, 19.4±2.9 mmHg after 1 week, 18.7±3.1 mmHg after 1 month, 18.5±2.6 mmHg after 3 months, and 18.3±2.7 mmHg after 6 months, p>0.05).
Conclusions: Our results show that after cataract extraction by phacoemulsification with IOL implantation ACD and ACA increased, but no significant changes in IOP occurred in patients with uveitic cataract. Although IOP values significantly decreased following uneventful phacoemulsification, we did not see any change in IOP values in our series. This may be due to the existence of inflammation in the trabecular meshwork and inflammatory debris. Key Words: Uveitis, cataract, phacoemulsification, inflammation.
