3Uz. Dr., Fırat Üniversitesi Tıp Fakültesi, Göz ABD, Elazığ, Türkiye Purpose: A literature review supported by 3 cases of ptosis after trabeculectomy and evaluation of surgical treatment methods of ptosis and surgical results.
Materials and Methods: Case 1: 65 years old female patient who underwent right eye cataract surgery 3 years ago, trabeculectomy 1.5 years ago had ptosis following trabeculectomy. Measurement of right eye?s levator function (LF) was 11 mm, palpebral fi ssure height (PFH) was 7 mm and margin-refl ex distance (MRD) was 1mm. Case 2: 62 years old phakic female patient who underwent left eye trabeculectomy surgery had 13 mm LF, 6 mm PFH and 1-1.5 mm MRD in the left eye. Case 3: 28 years old male patient who underwent right trabeculectomy with mitomycin C under general anesthesia had 16 mm LF, 4.5-5 mm PFH and 0-1 mm MRD.
Results: While first 2 cases had bilateral symmetry, third case had 1 mm of residual ptosis. None of the cases had over-correction or lagophthalmus and surgical outcomes are quite satisfactory.
Conclusion: Ptosis after glaucoma surgery is a complication which causes functional and cosmetic defects. In elderly patients who are decided to undergo surgery, it should be remembered that levator disinsertion can be seen and only suturing of disinserted levator aponeurosis to tarsus can be enough.
