³M.D. Professor, Bülent Ecevit Universit Faculty of Medicine, Department of Ophthalmology, Zonguldak/TURKEY In this case we reported a metallic fragment originated from aspiration cannula and dropped onto the iris during phacoemulsification surgery. A 53-year old patient with blurry vision on the right eye was planned to have cataract surgery. During the cortex removal of an uncomplicated surgery, a metallic fragment detached from the aspiration cannula and placed on the iris. The fragment was noticed and removed by a forceps after filling the anterior chamber with viscoelastic material. The second instruments used in phacoemulsification surgery should be controlled with certain intervals and the surgeon should inspect these instruments carefully before, during and after the surgery in order not to encounter such complications.
