3M.D. Professor, Adnan Menderes University Faculty of Medicine, Department of Ophthalmology, Aydın/TURKEY Purpose: To test the intraocular pressure (IOP) effect of the Ex-Press shunt implantation surgery in various types of glaucoma.
Materials and Methods: In this retrospective study 30 eyes of 29 patients with neovascular, pseudoexfoliative, chronic angle-closure, post-keratoplasty, üveitic, due to silicone, primary open-angle, ghost-cell, traumatic aphakic, inflammatory glaucoma were included. The eyes were complicated with diabetic retinopathy in 10 eyes, a history of anterior uveitis in 3 eyes, the previous retinal detachment surgery in 1 eye, traumatic retinal detachment in 1 eye, traumatic vitreous hemorrhage and lens dislocation in 1 eye, trauma in 3 eyes, central retinal vein occlusion in 1 eye and degenerative myopia in 1 eye. Phacoemulsification surgery was performed to 7 eyes (23%) simultaneously. Complete success was defined as IOP≤18 mmHg without glaucoma medications, limited success was defined as IOP≤18 mmHg using glaucoma medications, failure was defined as despite the glaucoma medications IOP>18 mmHg or additional glaucoma surgery is required to be assessed.
Results: The average follow-up was 12.4±7.1 (range 1-30) months. The mean preoperative IOP was 39.3±12.8 mmHg and significantly decreased to 16.4±10.8 mmHg in postoperative period. (p<0.000). At 12 months folllow up complete success rates were 57%, limited success were 23%, failure were 20%.
Conclusion: Ex-Press shunt implantation surgery had 80% full or limited IOP control rate success in most complicated glaucoma eyes with an average of 12 months follow-up.
