5M.D. Professor, Atatürk Training and Research Hospital, 1st Eye Clinic, Ankara/TURKEY Purpose: To investigate the effects of cataract extraction on SITA perimetry in primary open angle glaucoma (POAG) patients with a glaucomatous visual field defect.
Materials and Methods: Twenty three eyes of 16 POAG patients who had uncomplicated cataract surgery with phacoemulsification between 2005 and 2009 were included in this retrospective study. Best corrected visual acuity (BCVA) was measured with the Snellen test, intraocular pressure was measured with Goldmann tonometry and the visual field was analyzed with the SITA-Standard 24-2 test at the Humphrey visual field analyzer, both preoperatively and postoperatively. Visual field parameters, MD (Mean deviation) and PSD (pattern standard deviation) were also recorded.
Results: The mean BCVA value of the patients was 0.25±1.72 (0.05-0.7) preoperatively and 0.74±0.25 (0.3-1) postoperatively. Postoperative BCVA was significantly better than preoperative BCVA (p=0.0001). The preoperative mean MD value was -8.56±5.42 (-3.17--24.77) and mean PSD value was 5.03±3.36 (1.81-14.31). Postoperatively these values were -6.41±4.04 (-1.41--20.33) and 4.76±2.43 (1.53-9.57) respectively. The mean postoperative MD value was significantly better than the postoperative one (p=0.002) although PSD values showed no significant difference (p=0.681).
Conclusion: PSD is considered to be a more reliable parameter in the glaucoma follow up of patients with cataract, as we observed a considerable improvement in the MD in conjunction with increased BCVA after cataract surgery of the patients who had cataract and PAAG, whereas the PSD value remained unchanged.
