Alternative Surgical Approach to Subluxated Cataracts

Materials and Methods: This study comprised of 152 eyes of 135 patients who has been diagnosed as subluxated cataract. Capsular tension rings were implanted in 118 eyes and Cionni ring (double looped) in 26 eyes, while in 8 eyes Ahmed capsular tension segment was implanted. Surgical technique and results were discussed.
Results: In 118 eyes with capsular tension ring implantation, 3 (2%) had progressive intraoperative zonular dialysis and 1 (0.8%) had capsular rupture, in these 4 eyes, capsular tension ring was extracted and scleral fixation intraocular lens was implanted. In 9 (7%) of the 114 eyes with capsular tension ring, postoperative progressive dislocation of the capsular tension ring was observed and these eyes has undergone scleral monofixation procedure(on one haptic). In 26 eyes with Cionni ring implantation, capsular stabilization was established. Also in 8 eyes with Ahmed capsular tension segment, no capsular instability was seen. In eyes with capsular stabilization, intraocular lens was well-centered geometrically. In 118 eyes with capsular tension ring implantation, postoperatively 5 (4%) eyes had high intraocular pressure that responded well to medication.
Conclusion: In case of subluxated cataracts, Cionni ring or Ahmed capsular tension segment can be alternative to capsular tension ring for better stabilization of the capsular bag and better santralization of the intraocular lens.
