The Effect of Unilateral and Bilateral with Short Interval Phacoemulsification Surgery on Macular Thickness

3Selçuk Üniversitesi Meram Tıp Fakültesi, Göz Hastalıkları, Konya, Prof. Dr. Purpose: To compare uncomplicated, unilateral phacoemulsification with uncomplicated, bilateral phacoemulsification with short interval in terms of macular edema development.
Materials and Methods: Among cases who were planned to undergo phacoemulsification and intraocular lens (IOL) implantation due to senile cataract, 37 were grouped as “unilateral” and 13 cases whose fellow eye underwent phacoemulsification and intraocular lens (IOL) implantation within 4 weeks were grouped as “bilateral”. Ophthalmological examination and central macular thickness (CMT) measurement with optical coherens tomography (OCT) were performed to all cases preoperatively and at the postoperative 1. and 7. days, 1 and 2 months.
Results: The initial mean visual acuity was 0.82±0.89 in the “unilateral” group and increased to a level of 1.0 at the 1st month. The CMT increased from 199.27±20.68 m to 212.86±21.28 m at the 1st month in this group which was statistically significant. In the “bilateral” group the initial visual acuity of 0.48±0.74 increased to 1.0 at the 2nd month. The mean CMT of 192.30±15.40 m preoperatively demonstrated a statistically siginificant increase to 202.10±16.31 m at the 7th day and to 220.09±28.76 m at the first month. The change of CMT between preoperative and 1st month measurements was 12.85±21.17 m for the unilateral group and 20.40±10.81 m for the bilateral group.
Conclusion: The phacoemulsification without complication gave rise to an increase of CMT at the first month even there were no risk factors for cystoid macular edema and the visual acuity demonstrated no statistically siginificant change. The bilateral phacoemulsification with short intervals caused more increase of CMT at an earlier period. Keywords : Cataract, macular edema, optical coherens tomography
