Intraocular Lens Implantation in Traumatic Cataract Cases and Visual Outcome

3S.B. Ankara Numune Eğitim ve Araşt. Hast. 2. Göz Kliniği, Ankara, Şefi, Doç. Dr. Purpose: To analyze the postoperative visual outcome and complications after cataract surgery with intraocular lens (IOL) implantation in traumatic cataract cases.
Materials and Methods: We retrospectively reviewed the files of 40 traumatic cataractous cases who underwent cataract surgery. Patient’s age, trauma reasons, time between trauma and surgery, complications and postoperative visual outcome were recorded. Traumatic cataract was removed by phacoemulsifcation in 8 cases, by extracapsular cataract extraction in 30 cases, by intracapsular cataract extraction in 1 case with phacomorphyc glaucoma and by pars plana lensectomy in 1 case with subluxation. A posterior chamber (8 foldable, 19 polymethylemetacrilatte-PMMA, 6 scleral fixated) IOL implantation was performed in 33 (82.5%) cases. An anterior chamber IOL implantation was performed in 3 (.75%) cases. We couldn’t be able to implant any IOLs in 4 (10%) cases.
Results: 33 (82.5%) patients were male and 7 (17.5%) patients were female. Mean age was 49.0±18.5 (3-77 years). Patients were followed-up for mean 14.4 months (ranged from 6 to 24 months) after surgery. Traumatic cataracts were caused commonly by penetrating type of trauma. Visual acuity increased (1-10 Snellen lines) in 8 (100%) foldable IOL, in 12 (63.1%) PMMA IOL, in 4 (66.6%) scleral fixation IOL and in 1 (33.3%) anterior chamber IOL implanted patients. Visual acuity decreased (2-7 Snellen lines) in 2 (11.1%) PMMA IOL and 2 (66.6%) anterior chamber IOL implanted patients. The most common postoperative complication was fibrinous uveitis.
Conclusion: The visual outcome of traumatic cataracts was related to severity of trauma, zonnular and capsular support existence. IOLs in the capsule are associated with favorable visual outcome and a low rate of postoperative complications. Keywords : Traumatic cataract, IOL implantation
